By filling out this form, you register to attend the ESO workshop "Astronomy for All: Advancing Diversity, Equity, and Inclusion within Chilean astronomical community", from January 20-21, 2025 and sign up for the mailing list of participants of this workshop. For ESO to organise the conference, a set of personal data is required from each participant.
