                  APPLICATION FOR FUNDS TO ATTEND
   LIBRARY AND INFORMATION SERVICES IN ASTRONOMY V
     HARVARD-SMITHSONIAN CENTER FOR ASTROPHYSICS &
     MASSACHUSETTS INSTITUTE OF TECHNOLOGY LIBRARIES
                                  CAMBRIDGE, MA, USA
                                      June 18 - 21, 2006


The Friends of LISA V (FOL V) Committee is working to raise funds from
publishers, institutions and individuals to assist those librarians from
developing and resource-poor countries who wish to attend LISA V but are
unable to receive full assistance from their institution. We hope to
collect sufficient funds to assist with housing in Cambridge, provide a
small per diem, assist with the registration fee and, in some cases,
assist with travel expenses.  Applications for financial aid should be
received by 1 December 2005.  Decisions will be made by 15 December 2005.

Housing and per diem assistance (in US$) will be given to recipients
upon arrival at the conference; registration assistance will be paid
directly to the conference organizers.  In cases where we are able to
provide travel assistance, it is our intention to send the funds via
wire transfer to your institution's bank before the end of April.  If
you are applying for travel assistance, please provide the requested
bank routing information on the application form below.

Applications for FOL V support may be sent via e-mail, fax or postal
mail, but E-MAIL OR FAX ARE PREFERRED.  We will acknowledge the receipt
of all applications, so if you do NOT receive an acknowledgement please
